Transfer Credit Appeal or Reassessment Request (Undergrad)

Who should fill out this form?

Fill out this form if you have received your interim or final Transfer Credit Results email and you would like to:

1) Submit an appeal based on your results not meeting your expectations and have additional supporting documents to submit.

or

2) Submit a reassessment request based on officially changing your major.

Appeals will be accepted within 6 months of receiving your transfer credit results. Reassessment requests will be accepted within 6 months of changing your major (March 31 deadline).
